Pain Management: referral from the primary care provider is generally required. This is because pain conditions can be complex and involve different body systems. All referrals are reviewed for appropriateness and completeness and should include: 

  • condition or body part (ie, low back, neck, knee) for which the patient is being referred
  • current demographics and insurance / facesheet
  • medication list, past medical and surgical history (if not included in office note)
  • duration of pain condition
  • a recent office note pertinent to the pain condition
  • recent imaging pertinent to the pain condition
  • treatment notes from specialists involved in the case such as neurosurgery, orthopedics, physical therapy etc.

Referrals that are missing this requested information will result in a delay in scheduling.

Our goal is to decrease pain and improve function. When this is achieved, patients are referred back to the primary or referring provider for ongoing care with the expectation that long-term care (including medications) will be continued by the primary care provider. This allows room in our schedule for new patients. We remain available for consultation as needed. Please be advised that our office will not initiate controlled substance medications for any new patients. We do not assume prescribing controlled substances for new patients.

EMG referrals should include: demographics / insurance, extremities to be tested, recent office note pertaining to these extremities and condition.
